Wisconsin Immunization Registry (WIR)
WIR is a computerized information system for the state of Wisconsin that collects vaccination histories and helps ensure correct and timely immunizations.
The Wisconsin Immunization Registry is accessible to immunization providers, both public and private, health management organizations (HMO), public and private schools, Wisconsin Division of Public Health employees and their authorized agents and Women, Infants and Children (WIC). All WIR users and their employers must sign an agreement to comply with the confidentiality agreements.
What are the benefits of signing up for the WIR?

- Assists providers in determining which immunizations are needed and ensures children get only the vaccinations they need
- Consolidates immunization records from different sources into a single record
How can I participate in the WIR?
You can request that your child's immunization records be entered into WIR at your health care provider?s office.
Do I still need a personal record of my child?s immunization?
Yes, you still need that personal record of your child's immunizations. This card should be brought with you to all medical appointments. Whenever your child receives vaccinations, make sure your provider updates your child's record.
