Where to Get Shots - Immunization Services Schedule

THIS SCHEDULE WILL BE FOLLOWED EACH MONTH

Children must be accompanied by a parent or legal guardian. If someone other than a parent or legal guardian is bringing a child under 18 years of age to the clinic, the proper Kenosha County Division of Health and State of Wisconsin immunization forms must be filled out by the parent ahead of time.

PLEASE BRING ANY RECORDS OF YOUR CHILD'S IMMUNIZATIONS.
NO RECORDS - NO IMMUNIZATIONS.

A $8.00 administration fee per immunization is requested.
